Why do we ask for detailed information about you?
We ask you for some personal information about your experience of health conditions /treatments /services /communities as a patient, service user or carer. We only use this information to match your experience with the topic of a research application or research brief. By providing this information, you consent to us using it for this purpose. Please do not feel you have to answer all the questions, but it would help us to find suitable research applications for you to review.
What will happen to your information?
Your personal information is held and used in accordance with the General Data Protection Regulation 2016 (GDPR) EU 2016/679 and the Data Protection Act 2018. At the NIHR, your personal information is securely held in a management information system. The Government's Department of Health and Social Care (DHSC) is the Data Controller under GDPR.
The NIHR respects the privacy of individuals who share their information and process it in a manner that meets the requirements of the GDPR. More details, including information about your rights, can be found in the NIHR´s Privacy Policy.
Who has access to your data?
Only those employees at the NIHR Coordinating Centre responsible for processing your personal information and managing the public review process will have access to your information.
Your information may also be used for statistical analysis purposes. We may share non-identifiable summarised data with the DHSC. We do not share your information with any other third parties.
How long do we keep your information?
We will keep the information you provide until you inform us that you do not wish to continue as a public reviewer. You can ask for your information to be updated or removed at any time. If not instructed otherwise by you, we will contact you in 12 months to check if you wish your information to be updated or removed.
How can you ask for your information to be removed?
If you are no longer interested in being a public reviewer, you can ask for your information to be removed at any time by emailing us at publicreview@nihr.ac.uk.

You do not have to complete this form all at once. You can start by entering your email address and completing all the required fields marked with an asterisk (*). Enter a password and then click on the ´Save and Submit´ button at the end of the form. You will then receive an email confirming that we have received the form.
