Equality, Diversity and Inclusion Reporting Form - committee membership applicants

The NIHR is committed to promoting equality, diversity and inclusion in research. Although it is mandatory to answer these questions, it is possible to select "prefer not to say" as a response. However, the more information you provide, the more effective our monitoring will be.

This information will be held separately from application forms, and stored anonymously and securely. Those shortlisting and appointing will not see this information, and it will not be used to to make decisions about any application you make to NIHR.
